Per il contesto sono in congedo per malattia a causa della depressione, sono già passati alcuni mesi.
Quindi ho ricevuto questa lettera e ho intenzione di fissare un appuntamento con la mia psicologa (è del mio paese e le visite sono online), ma lei mi ha detto che ha bisogno di sapere se ha bisogno di scrivere il rapporto in un certo modo, ad esempio se ci sono alcune cose e argomenti di cui deve parlare, o può semplicemente fare una valutazione generale sul mio stato psicologico? L’ho cercato, ma non sono riuscito a trovare nessuna informazione a riguardo. Oppure dovrebbe scriverlo la mia Hausartz perché è lei che mi scrive il Krankmeldung?

Not an expert in medical bureaucracy, but I think this needs to be communicated with the health insurance.
As I witnessed those things over the years, doctors (in the German system) are very much in the know how these standrad procedures for German health insurances work and just do it automatically in the right format with the right details and so on.
When the doctor is not in that system, I would make sure I get everything I can from the health insurance in terms of information and rely that to the doctor or have them doing it.
