Dobry den, I am a medical student in Kosice. My parents wanted to send me a package and I recieved this in the post. I am unable to understand what this procedure is even after translation. Any help would be appreciated

Also, my condolences. Dealing with Slovak customs is bureaucratic pain in the ass.
