      Talking to a consultant friend, he told me that good/ambitious consultants/surgeons do not want to work in small local hospitals. They want to work in Dublin or Cork at a push, and probably Limerick would count if it wasn’t so badly run.

      The TLDR is that the people who end up working in small local hospitals are (often, not always) the less good, the alcoholics, the people that are burnt out, etc. etc. – this is why the clinical outcomes in these hospitals is so much worse than in the bigger urban hospitals.

      Now the government has known this for a while – this is why they have tried to create centres of excellence, and treat things like cancer in the large, good hospitals with good medics. But local people and politicians do not understand why they are doing this, and the government (successive governments, in fact) cannot come out and say ‘top doctors do not want to live in Waterford or Donegal’.

      So they say nothing, and people fume.
