Qualcuno può illuminarmi su che tipo di emergenza medica creerebbe una situazione in cui le persone è consentito salire ma non scendere dal treno??

These two things are not related. This S9 journey starts at Essen Hbf today so technically there are no passengers to exit the train anyway.

Enter/exit only stops are commonly used to prevent people from using long distance trains for short journeys between stations within the same local area. There is nothing that physically stops passengers from entering/exiting at such a stop, but it prevents the train from showing up in the search results when looking for connections and requires a ticket from/to the previous/next regular stop. A ticket with an enter only stop as destination would be invalid because in theory you can’t exit there (if it would be valid for the different train category in the first place).
I’m not sure what’s the intention of using it for an S Bahn. Because it’s unplanned, the entire ticket thing obviously doesn’t apply. They might want to prevent the station from overcrowding after an emergency, making people who would have left the train on one of the cancelled stops go to the next station before turning around. But since it’s an S Bahn, many people will know where to switch trains without looking up connections, so they’d also have to make announcements like “Please switch trains at the next station, it’s too crowded here”.
