The number and address are correct. Why don’t you give them a call? Why should we know more than DAK itself?
dont know if legal or illegal scam. But its possible that you were tricked into something (like you have to opt-out not opt-in).
You need to talk to the DAK to find out what happened
Maybe some shady doctor‘s office just handed it to you to sign? Happened to me once but I refused to sign it because they couldn’t explain what my advantages would be
