The *Beglaubigung* is issued by your local *Gesundheitsamt*. The office may have a slightly different name, depending on what state (*Bundesland*) you live in. Google “BTM Beglaubigung <your city/*Landkreis*/…>” to find the competent authority and an explanation of the exact process.
They might require the prescription to be issued by a doctor from the same state.
