It means you breastfed your baby. Did that discussion come up?
The “6-fach” in the Bemerkungen is probably related to the 6-fach Impfung (vaccination).
Second line: ? -> Vitamin D!
No idea what the first one means. Second image seems to talk about the hexavalent vaccine (maybe it’s ‘ungewünscht’, meaning you were against giving your baby the hexavalent vaccine?), then something about Vitamin D.
