What other errors did he do?
A lot of theses issues arise from poor board oversight. What questions if any where asked re finances , external reviews , why did the auditors not catch this ?

White had failed to mention the Charities Regulatory Authority (CRA) had been in touch with Vision Ireland, with serious questions being raised. At a board meeting in May last year, White had to apologise for the omission. The CEO said the regulator had written to the charity on March 25 – almost a whole month before the crisis financial meeting – “seeking some clarification on issues”, followed by a more detailed letter in May.The HSE is also investigating allegations of financial and corporate governance.
