Kde je záchod = where’s the toilet
Kde jsou toalety = where’s the restroom/lavatory
“Kde je záchod” would be a more precise/literal translation, but this works, too.
Yes, but correct is singular form too: Kde je toaleta?
toalety is restroom – whole room
zachod is toilet – just the one
jsou is plural of je because toalety in czech is plural – like for example many toilets in that room
The sentence is correct in my opinion. It’s used here like that. But no problem if you’ll ask “Kde je zachod”. People will understand too. But in that case it will be “Where is the toilet”
The word toalety sounds a bit more formal to me, I would choose “záchody” instead. Btw Chatgpt might be useful for explaining grammar.

Both “Kde jsou toalety” and “Kde je záchod” are ok. The first one is more formal than the latter and is often used for public restrooms, I’d probably say that while shopping or in a restaurant rather than “Kde je záchod” (both are fine though). If you’re at someone’s place though you’d most likely ask “Kde je záchod”.
You can you bought “Toalety” is a formal way to say “záchod.” If you ask a pal, go for Záchod. If ask a staff in restaurant says Toalety.
